Care Advocate

Position Summary

  • Location: Remote

As a Care Advocate to support our members to have the best possible experience at Joshin. You will play a key role in optimizing both our member’s and our caregiver’s experience and will be assigned members based on your expertise and you will be with them through their specialized care journey. You will work closely with the Operations Team to understand the organization’s mission, strategy, principals, policies, and procedures in order to effectively communicate with members and walk them through the matching process.

What You Will Be Doing

  • Own and maintain relationships with families and their care teams, acting as their primary point of contact using your expertise, skills, and judgment to advise, research, coordinate, and “get done” any related tasks
  • Respond within 24-hours to conduct an initial intake and schedule a call to assess the members needs and goals
  • Outline member care and wellbeing goals and coordinate specialists and caregivers to meet their goals and receive the care they need
  • Ensure members Care Plan is the most up to date by creating check in cadences
  • Document accurate and complete notes of all family and caregiver interactions
  • Ensure caregiver and specialists availability is up to date to best consider referrals
  • Meet with the Operations Team regularly to report on issues or insights that helps optimize our processes, and receive updates on new customers and/or priorities

Who You Are

  • Bachelor’s degree required, healthcare-related and/or advanced degrees are a plus
  • 2+ years work experience in healthcare industry, clinical practice, care manager, case management, telephonic nursing, care coordination, patient/family education preferred
  • Show expertise in at least one of the following: insurance, HR Benefits, disability and/or neurodivergent care, health care plans, Medicare and Medicaid regulations
  • MA, LPN, RN encouraged to apply
  • Knowledge of and ability to navigate the healthcare sector
  • Strong customer service skills – curious, approachable and a deep desire to meet the care needs of our members
  • An unwavering commitment to reflect the mission of the company
  • Proficiency in more than one language is a plus
  • Previous experience using a CRM, HubSpot preferred
  • Empathetic and kind in written and verbal communication
  • Effective time management skills and high attention to detail
  • Superior organization and multitasking capabilities
  • Goal-driven, problem solver
  • Strong ethical standards and a respect for confidentiality
  • Ability to integrate and collaborate well with others while adapting to evolving service experiences
  • Remote work experience preferred
  • Proficiency with macOS systems, GSuite, and Slack
