Position Summary

A company is only as good as its people. Joshin is looking for a Care Coordinator to support our members to have the best possible experience at Joshin. As Care Coordinator, you will play a key role in optimizing both our members and our caregivers experience. Not only is the Care Coordinator the first point of contact for members who are seeking specialized care, but you will also facilitate the matching process to create caregiver-member dyads. Joshin takes the pain out of finding the right caregivers – screening, onboarding, scheduling caregivers in their area, and much more. You will work closely with the Operations Team to understand the organization’s mission, strategy, principals, policies, and procedures in order to effectively communicate with members and walk them through the matching process.

What You Will Be Doing

  • Help members using your expertise, skills, and judgement to advise, research,coordinate, and “get done” any related tasks
  • Support with improving and updating Joshin guides and Community Resources
  • Other non-client-specific work including, but not limited to: supporting other service team members, feedback and suggestions related to Joshin’s product, service, marketing, and guides
  • Respond within 24-hours to conduct a short initial intake
  • Assess the members wants/needs, gather necessary relevant information, outline, and answer questions about the process
  • Match the member by entering their information into their Care Plan, which will provide two potential caregiver options, or do so manually
  • Ensure caregiver availability is up to date to best consider referrals
  • Review caregiver bios, videos, and other records to understand their matching qualifications
  • Track all leads, regardless of whether or not the member follows through with a phone consultation and/or first session
  • Meet with the Operations Team regularly to report on referrals, optimize the process, and receive updates on new customers and/or priorities

What You Should Have

  • Bachelor’s degree required, healthcare-related and/or advanced degrees are a plus
  • 2+ years work experience in healthcare/caregiving
  • Proficiency in more than one language is a plus
  • Empathetic listener
  • Clear communicator
  • Well-versed in clinical specialties and modalities
  • Technologically adept
  • Strong organizational skills
  • Detail-oriented
